الفشل الكلوي (Renal failure)
للكلى دور رئيسي في تنقية الدم، إزالة الفضلات من الجسم، إنتاج خلايا الدم الحمراء والحفاظ على توازن السوائل، المعادن، الأملاح والأحماض الأساسيّة. أمّا الفشل الكلوي فهو فشل أو خلل في عمل الكلى. نتيجةً لهذا الفشل لا يتم تصفية السوائل من الجسم بشكلٍ صحيح، وبذلك تتراكم الفضلات في الدم ويَنخفض مُعدّل التبوّل.
هناك ثلاثة أنواع من فشل الكلى:
1. الفشل الكلوي المُزمن (CRF- Chronic Renal Failure): فشل في عمل الكلى الذي يتطوّر بشكلٍ تدريجيّ وبطيء. الفشل الكلوي المُزمن هو حالة لا رجعة فيها والتي تُؤدّي عادةً إلى الفشل الكلوي النهائي.
2. الفشل الكلوي الحادّ (ARF- Acute Renal Failure): فشل كلوي يتطوّر في وقت قصير يَستمر بين أيام لأسابيع. عادةً بعد القيام بعلاجه، تعود الكلى للعمل بشكل طبيعي، ولكن في بعض الحالات مُمكن حدوث ضرر لا يمكن إصلاحه الذي قد يُسبّب فشل كلوي مُزمن.
3. الفشل الكلوي النهائي (ESRF- End Stage Renal Failure): فُقدان وظيفي كامل للكلى يَتطلّب غسيل الكلى أو زرع كلى. ففشل الكلى يُسبّب احتباس السوائل وبقاء البروتينات داخل الجسم. ونتيجةً لذلك، تُصاب أنظمة الجسم ويحدث خلل في عمل أعضاء الجسم.
ومن أهم مُؤشّرات الفشل الكلوي هو ارتفاع نسبة مادّة الكرياتينين بالدم. حيث تُعتبر مادّة الكرياتينين مادّة سامّة يحذر ارتفاع نسبتها في الدم. مصدر الكرياتينين مادّة موجودة في العضلات على شكل فوسفات الكرياتين، حيث يتحول 2% من كرياتين الفوسفات إلى كرياتينين الذي ينتقل عبر الدم لكل الجسم وإلى الكلى ليخرج عن طريق البول. وبما أن الكرياتينين من العضلات فإن العلاقة بين تركيزه بالدم وحجم العضلات علاقة طرديّة، فكلّما كانت كتلة العضلات أكبر زاد تركيز الكرياتينين في الدم ولذلك فان نسبة الكرياتينين لدى الرجال (1.2-0.6 ملغم لكل ديسيلتر) وهي أكبر من نسبة الكرياتينين لدى النساء (1.0-0.5 ملغم لكل ديسيلتر).
فحص نسبة الكرياتينين: يُستخدم فحص نسبة الكرياتينين في الدم كدليل على صحّة عمل الكلى حيث أن زيادته في الدم ونُقصانه في البول تُعتبر علامة لوجود مشاكل في الكلى وبداية الفشل الكلوي أو الانسداد بها.
أسباب الفشل الكلوي
1. السّكري.
2. ارتفاع ضغط الدم الحادّ.
3. الاستخدام المُزمن لبعض الأدوية مثل: مُضادّات التهاب غير ستيرويدية (NSAID)، مُثبّطات مناعة.
4. تكيّس الكلى.
5. سوء تغذية.
6. التهابات أو عدوى جهازيّة.
7. أمراض المناعة الذاتيّة.
8. حصى الكلى.
9. العيوب الهيكليّة في المسالك البوليّة التي تُسبّب انقباض.
10. تناول أغذية غنيّة بالكولسترول والبروتين والدهن الحيواني.
11. مرض كلوي يُعيق عمل الكلى مثل: التهاب كبيبات الكلى،الانسداد الكلوي وموت الخلايا في أنابيب الكلى الصغيرة. أحد أهم أسباب الفشل الكلوي
12. جريان دم خفيف وبطيء في شرايين الجسم.
13. خلل في العضلات وإنتاج كميّة أكبر من الكرياتينين.
14. جفاف ناتج عن إسهال، تقيّؤ مُستمر، عدم شُرب الماء الكافي، ارتفاع درجة حرارة الجسم.
15. بذل جهد عالي ومُستمر وعدم أخذ قسط من الراحة.
16. التهاب في الجسم أو الجهاز البولي.
ومن المُمكن تصنيف المُسبّبات للفشل الكلوي الحادّ حسب مكان الإصابة:
1. قبل الكِلية (Prerenal): خلل في إمداد الدم إلى الكلى أو انخفاض حجم الدم نتيجة اضطرابات مثل فُقدان الدم، الجفاف، انسداد الأوعية الدمويّة، الحمى العالية المُستمرة، الإسهال الحادّ، التقيؤ، فشل القلب.
2. داخل الكِلية (Intrarenal): إصابة الأجزاء داخل الكِلية (نيفرون أو الكليون، وأنابيب الكلى، وغيرها) نتيجة فشل كلوي، التهاب، الاستخدام المُزمن للأدوية التي لديها فاعليّة سُميّة للكلى (مثل العقاقير المُضادّة للالتهاب الغير ستيرويدية، المُضادّات الحيويّة، أدوية مُتنوّعة لعلاج السرطان، أدوية مُختلفة لعلاج زرع الأعضاء)، تَسمُّم المعادن الثقيلة، استخدام المواد المُشعّة التي تُستخدم للتصوير، أو مُستويات مُرتفعة من حمض اليوريك.
3. خلف الكِلية (Postrenal): اضطرابات في المسالك البوليّة السفلى مثل تَضخُّم البروستاتا، تكوّن أورام تقوم بالضغط على المسالك البوليّة، القسطرة الدائمة، تضييق المسالك البوليّة بسبب الحجارة أو خلل شكلي.
أعراض الفشل الكلوي
1. ارتفاع مُستوى الكرياتينين في الدم.
2. ارتفاع مُستوى الكرياتينين في الدم.
3.تورّم (وذمة)– تجمُّع واحتباس السوائل في الجسم خاصّةً تحت العين وفي الأطراف.
4. بيلة دمويّة (وجود دم في البول).
5. ضيق التنفّس.
6. فُقدان الشهيّة. v
7. تعب مُستمر.
8. صُداع.
9. غثيان وقيء.
10. الشعور بالعطش والجفاف.
11. جفاف الجلد.
12. أعراض أمراض أخرى غير حصريّة مثل بول غامق اللّون، قلّة البول وغيرها.
